Low back pain is defined as pain and discomfort, localized below the costal margins (margin of the ribs) and above the gluteal folds (fold of the buttock), with or without leg pain. Low back pain is a major health problem throughout the world. It is one of the leading causes for disability which interferes with quality of life, work performance, sports performance and general well-being. It cannot be termed as a disease because most of the time the cause and symptom exhibited is non-specific.In Indian population the prevalence of low back pain has been found to range from 6.2% to 92 % with increase of prevalence with age and female preponderance (Bindra et al., 2015).
Low back pain is one of the most researched areas in the musculoskeletal health science. A study on worldwide research productivity in the field of back pain shows there were more than 50,000 research articles published related to back pain in the past 20 years and about 600 research studies were from India (Wang and Zhao Medicine, 2018). But still the problem of low back pain remains a grey area because of the complexity of the structures in the low back region and also because the causes vary from a simple muscle cramp to inflammatory condition or degenerative condition or neoplastic disease or spinal fractures.
To keep it simple, based on the mode of presentation, the low back pain patients can be categorized into 5 groups. (i) Patients who have low back pain only, (ii)Patients who have low back pain with back stiffness, (iii) Patients who have low back pain with radiating pain to lower limbs, (iv) Patients who have low back pain with altered sensation in the lower limbs, (v) Patients who have low back pain with weakness of lower limbs. Pain is something through which our body tries to convey us that something is not right. It may be structural or psychological, but whatever may be the presentation, everything needs appropriate attention at the earliest.
Low back pain is mostly managed conservatively with exercises, medication and life style modification. Surgery is indicated only if there is a potential nerve damage or severe compression of nerve roots or worsening structural changes.
